Re-use Data from Previous Certificate

If you have previously requested a certificate for a fund, this page will allow you to preload the fund information and contact details previously provided for that fund into a new certificate request form.

This can be very useful where:

You need an actuarial certificate for a new financial year, when you have already requested a prior year certificate for that fund from Verus.

This page enables you to load the Fund Information and Contact Details sections of the online form from a previous certificate request, without re-typing any of that information.

You have requested an actuarial certificate from Verus before and you want to re-use the Contact Details section of the form for a different fund.

This page enables you to load just the Contact Details section of the online form from a previous certificate request, and use it for another fund.

To gain access to the previous data you will need to submit the reference for the previous certificate (shown in the bottom right corner on the first page of the actuarial certificate). An email with a security key will immediately be sent to the email addresses provided as part of the previous request. Once you have received the security key, enter it in the form below to load a new certificate request form with preloaded fund information and/or contact details.

Note that for data security and confidentiality reasons, the security email will only be sent to the primary and alternate email addresses you previously provided. If you no longer have access to these email addresses, please contact us via email (enquiries@verusactuaries.com.au) or phone 1800 978 328 to obtain your previous request data via a manual security check.
